Medical Marijuana Identification Card (MMID)

The Medical Marijuana ID Card Program is voluntary.  If you are interested in participating, please call for further information.  Applications are accepted by appointment only.

At your appointment you will be required to provide:

• An Application
• A form of identification
• Proof of residency
• Medi-Cal eligibility
• A copy of your current recommendation
