Targeted Case Management

Targeted Case Management (TCM) is a home visitation program that provides specialized nursing case management service to individuals in a define targeted population to gain access to needed medical, social, education and other services.
Our goal is to ensure that the changing needs of people are addressed on an ongoing basis and appropriate choices are provided among the widest array of options for meeting those needs.

Target Population

  • Children under the age of 21
  • Medically Fragile Individuals
  • Individuals in Jeopardy of Negative Health or Psycho-Social Outcomes
  • Individuals with a Communicable Disease
